Proposal of Histologic Grading System in Ischemia/Reperfusion Injury of the Small Bowel


Abstract
Histologic examination in ischemia/reperfusion injury of the small bowel is an important method to assess the damage in experimental models. We propose a histologic grading system that can be used in acute experimental setting of small bowel that
has an
ischemia/reperfusion background. Seventy-three histologic samples from experiment of ex vivo perfusion of isolated small bowels were analyzed. We made schematic pictures of sequential changes of villi on the basis of actual changes in villi to
deseribe
more detailed. From the sequential histologic changes of ischemia/reperfusion injury of the small bowel 1) configuration change (ratio of width to height) of the villi, 2) epithelial lifting, 3) extent of epithelial loss, 4) epithelial
disruptions.

loss of crypt cells, 6) changes in muscle layers, and 7) cellular infiltration in lamina propria were selected as parameters. Each score was made according to their effect on tissue damage. Presence of epithelial disruption(+)5 and loss of crypt
cells
(+3/+6) were set to have a higher score than another five changes that has a lower score of + 1. Total histologic damage scores are sum of each score of seven histologic finding. Advantages of this numerical scoring system are objective, easy to
make a
score although it is not a blinded study and has wider range of scores (0-24).
